The service provides:
A supported environment in which to promote independent living
An individual support plan to encourage a swift return to full home life
Support to help regain confidence and independence
Care staff onsite 24/7, please note that night staff provide emergency response only and not planned care
The service aims to:
Help you regain skills that may have been lost during your recent illness or injury
Prevent avoidable hospital admissions
Reduce/delay admission to residential care
Facilitate safe discharge from hospital or other bed based facility
Maintain or improve levels of independence
Promote recovery and support your continued independence at home
Prepare you to go home, this means encouraging you to do things independently such as cooking, cleaning and undertaking personal care, so that you are able to do these things when you return home.
Achieving our aims together:
An assessment with be completed by a Health & Social Care professional who will assess your abilities and set realistic goals
You will participate in your individual reablement programme as identified by the Occupational Therapist
Your progress will be reviewed during a daily meeting to plan your safe and timely return home
Support you to maintain things that are working well and you manage independently or with support from family e.g. shopping, collecting and ordering medication.
Your Showell Court Reablement team:
The team is made up of a number of professionals who will be involved in various areas of your supprt plan. The team may include:
The Showell Court managment team
Social Work Professionals, who are available if you require further support when returning home
Occupational Therapist (OT) who will suppport you on your journey to maximise independence
Support workers who will be your day-to-day support during your short stay at Showell Court
Direct links to other services and professionals, depending on your specific requirements
Facilities at Showell Court:
All one-bedroom apartments have a living area, kitchen and bathroom, and are brightly furnished as a home from home
Showell Court has excellent communal facilities including a restaurant, garden and a large communal lounge
A choice of catering options are availble depending on your goals and support plans also a meal voucher will be provided for our onsite restaurant for the first 48 hours thereafter food preparation will be supported as per your support plan. Tea time meal deals will need to collected before 2pm.
Showell Court has a laundry room, hairdressers and beauty room
All apartments are equipped with Assistive Technology, the three rings system uses motion sensors to help us understand your activity levels and movement. The system can also link into additional equipment such as falls sensors, door sensors and light sensors should you need them. This will be considered during your assessment for the service and where needed will be provided free of charge during your time at Showell Court.
Visiting and valuables
We want you to feel comfortable at Showell Court for your stay with us and invite you to bring along personal items for your apartment, should you so wish
Showell Court cannot be responsible for safekeeping any valuables unless you ask for them to be held in our safe
Family and friends are able to visit you at Showell Court, and it is important to your reablement that they do so.
The service criteria:
You live within the City of Wolverhampton
You have been assessed by a Health or Social Care Professional as requiring social care reablement
You agree to work with the team towards your reablement goals
You are an adult who can mobilise a distance of at least five meters (with or without aids)
Your immediate needs are unable to be met in your usual place of residence
Showell Court is registered with the Care Quality Commission.
