Childrens Community Nursing Service (CCNS)

The CCNS Provide specialist nursing support for children who have acute health problems, usually on a short term basis and usually following discharge from hospital and after request via GP or hospital consultant. Support may include clinical interventions for example: wound care dressings, injections, infusions, and enemas. This support may be given at their home, school or the Gem Centre, as appropriate.

Specialist Nursing support is also provided as part of a Multidisciplinary team (including physiotherapy, occupational therapists, GPs, Paediatric consultants etc) to support the child/young person and family/carer with long term conditions who have chronic health problems and/or disabilities, who often remain with the service on a long-term basis. Support given for example include epilepsy support, palliative care support, incontinence advice and pads, bereavement support, enteral feeding equipment and training.

These children are supported in schools and advice given in homes 9-5, seven days a week. For families who wish to access the service outside these arrangements specialist agreement via the continued care Team for children services will be required.

For some specific  conditions for example Cystic fibrosis, Diabetes, Asthma, Oncology  there are specific Nurses employed by the Royal Wolverhampton Hospital trust or GPs and we will inform your GP we have referred you on to these services.

The teams also provide support in two special schools Green Park and Penn Hall. This care is for children who have learning as well as physical disabilities. Special School Nurses are available during school hours.
