Community Stroke Specialist Service

We are a small multi-disciplinary team who follow up all stroke patients for 12 months post-stroke. We review patients at home within 6 weeks of discharge, and at 3, 6 and 12 months.

We review a patient’s recovery, ensure that they are receiving the correct support, review and address their risk factors and give advice and information (e.g. healthy lifestyles, driving, return to work, leisure activities).

We will signpost and refer to other services as necessary. We are the patient’s “single point of contact” for their post-stroke journey.

We visit patients in their own homes (or we can see them at West Park Hospital if they would prefer).

The service is run Monday to Friday 8.00am – 4.00pm but the office is not staffed.

A referral is required from a healthcare professional following a confirmed diagnosis of a recent stroke by a doctor or consultant.
