Have your say about services for people living with dementia

 

People living with dementia, their families and carers and professionals are being invited to help shape health, wellbeing and social care services in Wolverhampton.

The City of Wolverhampton Council is carrying out a Joint Strategic Needs Assessment which looks at the current and future health needs of the local population and which will inform and guide service provision.

As part of the assessment, the council is carrying out a survey about the support available for people living with dementia, to understand what services are currently being used and what people think of them, and to identify any unmet needs of people with the condition.

The findings will be used by the Joint Strategic Needs Assessment Steering Group to recommend ways in which services can be improved in the future.

The survey will take around 10 minutes to complete. All responses will be kept confidential and no identifiable information will be collected. There are three versions of the survey, one for people living with dementia available:
