The Continence Care Service

The Continence Care Service are based at Lower Green Health Centre. 

What we do:

The Continence Care Service is a nurse led service offering holistic assessment, advice, treatment and management of bladder, bowel and continence related issues. We serve adults of all ages, both male and female, within the borough who are registered to a Wolverhampton GP.  This service is provided within primary care clinics, domiciliary settings, residential care and nursing homes.

We manage a home delivery service for the provision of containment aids to clients who have been assessed and require ongoing management as part of their care. The service can only prescribe containment products (pads) that are provided by the appointed supplier that has been awarded the contract via the tendering process. The contact is currently with Ontex. All prescriptions provided are determined by evidence based clinical need.


Initial assessments may be performed via a telephone consultation or a face to face appointment dependent upon the clinical need. If the assessment indicates that there is a clinical requirement for a face to face appointment then this will be arranged to take place either at Lower Green Health Centre or at home. The Continence Care Service perform assessments in the environment that has the most potential for a successful outcome. 

If the patient has emergency continence needs (ie; receiving palliative care or skin integrity is compromised by incontinence) and is actively on the Community Nursing Caseload they will be initially assessed by Community Nurses as part of their holistic care. Following initial assessment, if further input is required, the Continence Care Service will support the patient.  

Children with bladder or bowel dysfunction are supported by Children's Services.

Referral Process

People who are experiencing symptoms of faecal or urinary incontinence or voiding difficulties should speak to their GP or any other health care professional involved in their care.

This is to confirm that the incontinence or voiding issues are not a symptom of another condition requiring further investigation. Once the medical assessment is complete the patient may be referred for a continence assessment. If the patient is an inpatient in a Wolverhampton hospital a generic continence assessment should be completed by the hospital nursing team during the stay and prior to discharge. 

If the patient has desire and potential for improvement and the nursing team are unable to succeed a referral to the Continence Care Service will be made. Health professionals can refer by emailing the Continence Care Service and requesting a copy of the referral table. 

Patients that are on the community nursing service caseloads and they have an urgent need for an assessment ie risk of skin breakdown or receiving palliative care will be assessed by the community nursing services as part of their holistic assessment process.
