Community Anticoagulation Service

 

The Community Anticoagulation service provides general anticoagulation management advice, blood monitoring, review and anticoagulation medicine dose guidance to patients who require Oral Anticoagulant Therapy

The service has a medical consultant lead and is comprised of clinical specialist nurses with support from clinical support workers and an administration team.

Anticoagulation Clinics

The service provides clinics from a number of locations across the city of Wolverhampton. The clinics operate from 8.30am – 4.30pm Monday to Saturday, excluding Bank Holidays

Domiciliary

Home visits are offered to patients who are clinically housebound only. Patients are continually assessed and as their condition improves will be offered clinic appointments.

Drop-in clinics

We provide a drop in service operated from the Community Phlebotomy Service on occasions when patients are unable to attend a clinic or require their anticoagulant blood level measured between appointments. Once you have received the result from your blood test you will contact the telephone advice service to obtain management guidance.

Drop-in clinics are often very busy and spaces are limited. If the drop- in sessions are full for the day you will be advised of another drop-in clinic close by or attend our next drop in. The drop in clinics are available from a number of locations across the city of Wolverhampton. See Community Phlebotomy Time table in Downloads section.

Telephone advice service

The service advice line offers the facilities from changing an appointment to obtain specialist advice from trained Anticoagulation Nurses.

A referral is required from your GP or Hospital doctor to ensure you receive on-going support throughout your treatment plan.
