Community Stroke Specialist Service

Our comminity service is intended as a source of information, resources and useful contacts for those affected by Stroke. We also have a rehabilitation (outpatient follow up clinic)

Our team reviews a patient’s recovery, ensure that they are receiving the correct support, review and address their risk factors and give advice and information (e.g. healthy lifestyles, driving, return to work, leisure activities). We cover the Wolverhampton area. We visit patients in their own homes (or we can see them at West Park Hospital if they would prefer).

The Carers Resources Section is intended to help provide information to help look after you, the carer and promote your health and wellbeing. This section has tips on looking after your own health including managing stress, anxiety and low mood.

Finally the Useful Contacts Section provides a list of services available within Wolverhampton covering a variety of areas relevant to Stroke and supporting Carers.

We will signpost and refer to other services as necessary. We are the patient’s “single point of contact” for their post-stroke journey.

Please note: West Park Hospital Does not have an Accident & Emergency (A&E) department
