Who can make referrals for assessment for equipment?

Initial referral must always come from a qualified health professional. Once known to the service, self referral.

How is assessment accessed?

Referral to Wolverhampton Wheelchair service.

How is need assessed?

All users receive an assessment by a member (or members) of the Wheelchair service team and equipment is prescribed according to need. There is a monthly seating clinic in partnership with a third party specialist seating company.

How long will the assessment take and when will be the equipment be provided?

Variable according to complexity but we aim to complete the episode within 18 weeks of referral being received by the service.

Will equipment be provided new or recycled?

Both but recycled wherever possible.

How is equipment provided?

Handover in monthly seating clinic, home, school or other service accessed by the user

Is demonstration or training required prior to use?

Yes.
