Chronic kidney failure occurs when the kidneys begin to work less efficiently. It is a serious, slow progressive disease which, if left unmanaged, will potentially result in end-stage renal failure and renal replacement therapy.
The team, based at New Cross Hospital, consists of Doctors, Specialist Nurses and Dieticians who work closely with patients, carers and primary care to manage the condition.
Our aim is to provide a service that continually works to improve the treatment, experience and outcomes for patients with renal disease.
Services offered:
Chronic Kidney Disease Team
Specialist Nurses provide individualised, supportive care to patients with chronic kidney disease, including patients who are preparing for dialysis therapy, transplantation, live kidney donation, conservative management and anaemia management.
Home Therapies Team
Patients with end stage kidney disease requiring dialysis and whom have chosen home dialysis are trained and supported to carry out their own dialysis at home. Patients may choose haemodialysis or peritoneal dialysis.
Haemodialysis Service
The service encourages patients to contribute to their haemodialysis treatment at these units by delivering a Shared Care programme providing training and support for those patients who wish to participate.
Dialysis away from base for patients visiting the area can be facilitated within the satellite units.
